  1. You must visit a USCIS designated Civil Surgeon only.
  2. Make sure the civil surgeon provides Form I-693 to you in a sealed envelope
    • Prior to sealing the envelope, you will be required to sign the form
    • Ensure all your information on the form is correct
    • Ask whether you can have a photo copy of the completed form before they give you the sealed copy
  3. Health Insurance may cover some cost of the physical
    • Some tests and vaccines such as X-Rays Td/Tdap etc. may be covered by your Health Insurance, so remember to check with the civil surgeon’s office
